NEW Weight Loss Enrollment
You will receive the necessary paperwork via email 24 hours before your scheduled visit. It is REQUIRED that you complete these forms by 8 AM on the morning of your exam. If the forms are not completed by this time, it will be considered a no-show. A cancellation fee of 50% will be charged for no-shows or cancellations made less than 24 hours before the appointment. Please note, there are NO EXCEPTIONS to this policy. Exam is good for one year. All physician exams are virtual.
